  1. Download Mindbody App and book your first class to become our client (no payment required).

  2. Fill out this form and make payments accordingly. 

  3. After verifying your payment, we will add the credits to your account and apply them to all unpaid bookings.

Rollover Request

  • If your credits expired, you can purchase a new package within one month to have expired credits converted into roll over credits with two month expiration.

  • You can roll over 1 class from a 5 class package and 2 classes from a 10 or 20 class package.

Suspension Request

  • You can suspend all active credits on your account with a $15 fee (can be waived by doctors note).

  • Suspended credits from the same package must be activated together.

  • Each package can only be suspended once. ​

  • Suspended credits will expire after one year. You can email us to reactivate your credits.
